Provides nursing assessments & consultations in care management of individuals/families; coordinates immunization clinics, tuberculosis clinics & TB follow-up; and works collaboratively w/ community health providers. Req. bachelor’s degree in nursing, valid Hawaii RN license & valid driver’s license. No experience for level II; 6 mos. public health nursing exp. for level III; 1-½ yrs. PHN exp. for level IV incl. 1 yr. comparable to level III in State service. Appropriate substitutions allowed.
