NOTICE

REQUEST FOR PROPOSAL

The County of Kauai, Department of Finance, is requesting proposals to provide administration and servicing of a FLEXIBLE EMPLOYEE BENEFITS PROGRAM for the County of Kaua`i.

Proposals shall be received no later than 2:00 p.m., HST, Tuesday April 28, 2009, in the office of:

Before any prospective Offeror shall be entitled to submit an Offer, the Offeror must, wherein applicable, comply with the provisions of Section 103-55, HRS, regulating wages and hours and working conditions of employees of contractors supplying services to governmental agencies. Accordingly, Offerors shall complete the wage certificate for service contracts as provided in the County’s Schedule “F”.

The successful Proposer shall comply with the tax clearance requirements of HRS 103D-328, and the Responsibility of Offerors, compliance document requirements, of HRS 103D-310(c) and HAR 3-122-112.

A non-mandatory pre-proposal conference is scheduled for Wednesday April, 1, 2009; 9:00am HST at the Finance-Accounting Conference Room in Suite 280, 4444 Rice Street, Lihu`e, HI 96766. All interested parties, contractors, subcontractors, and respective union representatives are invited to attend. The Director of Finance reserves the right to cancel this Request For Proposal and to reject any proposal, in whole or in part, when in the best interest of the County of Kaua`i.

This Request For Proposal shall be governed by the requirements of HRS, Chapter 103D and the respective Hawai`i Administrative Rules. Copies must be obtained from the Division of
