YOU ARE HEREBY NOTIFIED THAT State of Hawaii, Department
of Commerce and Consumer Affairs, Plaintiff, has commenced an action in Civil No. 08-1-1795-09 (Victoria S. Marks) for damages suffered against you in the above-entitled Court wherein it requests judgment by way of an injunction, civil fines and restitution in an amount in excess of $10,000.00 against you, together with such other relief as the Court deems just.
YOU ARE HEREBY SUMMONED to appear before the Honorable Victoria S. Marks, Judge of the above-entitled court, at 777 Punchbowl Street, Kaahumanu Hale, Honolulu, Hawaii, on Tuesday, the 16th day of December, 2008, at 9:00 o’clock a.m., of said day, or to file an answer or other pleading and serve it before said day upon Denise P. Balanay, Plaintiff’s attorney, whose address is 235 South Beretania Street, Ninth Floor, Honolulu, Hawaii 96813. If you fail to do so, judgment by default will be taken against you for the relief demanded in the complaint.
