NOTICE OF HEARING

NOTICE IS HEREBY GIVEN that pursuant to Hawaii Revised Statutes (“HRS”) Chapters 91, 269, 269E, and 271, and Hawaii Administrative Rules (“HAR”) Chapters 6-61, 6-62, 6-68, and 6-80, the HAWAII PUBLIC UTILITIES COMMISSION (“commission”) will hold order to show cause hearings to determine (1) whether the operators listed below, who are subject to the One Call Center law provided in HRS §269E, should be fined or their failure to pay a registration fee and/or ticket assessment fees pursuant to HRS § 269E and Decision and Order No. 23086, and (2) whether the certificates of authority of the telecommunications carriers listed below, certificates of registration of other telecommunications carriers listed below (i.e., commercial mobile radio service and pay telephone providers), and the certificates of public convenience and necessity of wastewater utilities, motor carriers of passengers, and motor carriers of property listed below should be suspended or revoked for their failure to do one or more of the following: (a) file annual financial reports (“AFR”) pursuant to HRS § 271-25 and HAR § 6-80-91; (b) pay annual fees and/or assessed penalties, pursuant to HRS §§ 269-30, 271-27(i), 271-36, and HAR §§ 6-62-24(a), 6-62-42; (c) pay civil penalties pursuant to HRS § 271-27 and HAR § 6-68; and/or (d) pay an annual contribution to the telecommunications relay services fund, pursuant to Order No. 20193, filed on May 23, 2003, and as amended by Decision and Order Nos. 21049, 21847, 22536, and 23481.

The commission directs the PROPERTY AND PASSENGER MOTOR CARRIERS, T ELECOMMUNICATIONS CARRIERS, WASTEWATER UTILITIES, AND OPERATORS SUBJECT TO THE ONE CALL CENTER LAW listed below to appear before the commission at hearings to be held in the Commission’s hearing room, 465 South King Street, Room B-3, Honolulu, Hawaii, at 9:00 a.m., Tuesday, November 20, 2007.

Each entity listed below may be represented by an attorney of its own choosing. Sole proprietors may appear on their own behalf; a partner may represent a partnership; and an officer or authorized employee of a corporation, trust, or association may represent the corporation, trust, or association. If an entity listed below fails to appear at the scheduled hearing, the commission may revoke that entities certificate or assess a fine, if applicable.
