NOTICE TO OFFERORS
REQUEST FOR PROPOSALS
RFP No. HTH 07-DBC-02

Pursuant to Chapter 103D, HRS, The State of Hawaii, Department of Health (DOH), Solid Waste Branch, is soliciting a Request For Proposal (RFP) No. HTH 07-DBC-02 for establishing new Hawaii Deposit Beverage Container Program certified redemption centers or improving operations in existing centers. Specific criteria are listed in the RFP. Offerors must submit proposals in the format specified in the RFP in order to qualify for award.

One (1) double-sided bound original and three (3) copies of the proposal must be received (whether mailed or hand-carried) and time stamped by 4:30 p.m., (HST), May 25, 2007, at the Department of Health, Solid Waste Branch, 919 Ala Moana Blvd., Room 212, Honolulu, Hawaii 96814-4920. Proposals received after the deadline set for receipt of proposals will not be accepted. Faxed or emailed proposals will not be accepted. The Department of Health reserves the right to cancel this RFP and to reject any and all proposals in whole or in part when it is determined to be in the best interest of the State.

The RFP will be available for pick up in hard-copy form beginning April 16, 2007 from 8:30am – 4:30pm, H.S.T. at the below address:

However, it is your responsibility to call the RFP Contract Administrator at (808) 586-4226 as soon as possible to be placed on a mailing list for notification of RFP changes and/or issuance of addendums if your are planning to submit a proposal. The State will not be responsible to notify you of any RFP changes or addendums if you are not on the mailing list. Failure to receive any addendum shall not relieve you from any obligation under the RFP as submitted.
