YOU ARE HEREBY NOTIFIED THAT the State of Hawaii, Department of Commerce and Consumer Affairs, Plaintiff, has commenced an action against you in Civil No. 05-1-1702-09 (RKOL) in the Circuit Court of the First Circuit, State of Hawaii, for damages suffered against you in the above-entitled Court wherein it requests judgment by way of an injunction and civil fines against you, together with such other relief as the Court deems just.
YOU ARE HEREBY SUMMONED to appear before the Honorable Randal K.O. Lee, Judge of the above-entitled Court, at Kauikeaouli Hale, 1111 Alakea Street, Courtroom 5C, Honolulu, Hawaii, on Tuesday, the 22nd day of August 2006, at 9:00 oclock a.m., of said day, or to file an answer or other pleading and serve it before said day upon Diane R. Corn, Plaintiffs attorney, whose address is 235 S. Beretania Street, 9th Floor, Honolulu, Hawaii 96813. If you fail to do so, judgment by default will be taken against you for the relief demanded in the complaint.
