NOTICE OF SOLICITATION OF PROPOSALS
FOR HEALTH AND HUMAN SERVICES

The Department of Human Services, Vocational Rehabilitation and Services for the Blind Division is requesting for the following service:

1. Psychological Assessment Services: Including cognitive and personality assessment of individuals with disabilities, which may include neuropsychological assessments.

Applicants must be licensed psychologists in the State of Hawaii. Multiple contracts will be awarded under this request for proposal. The orientation for this Request for Proposal (RFP) will be held on Monday, May 1, 2006 from 10:30 a.m. to 11:30 a.m. at 600 Kapiolani Boulevard, Room 301. All interested parties are invited to attend.
