YOU ARE NOTIFIED that the Board of Dental Examiners will conduct a hearing in accordance with Hawaii Revised Statutes (hereinafter “HRS”) Chapters 91, 436B, 447, and 448 as well as Hawaii Administrative Rules (hereinafter “HAR”) Title 16 Chapter 79 to determine whether or not YVONNE S. HAIL’S dental hygienist license should be revoked, suspended, or otherwise disciplined, and, whether monetary fines and any other relief authorized by law, should be imposed against it. The Petition for Disciplinary Action Against Dental Hygienist License, filed with the Office of Administrative Hearings on or about April 24, 2017, alleges violations of the provisions of HRS Chapters 436B; 447; and 448 as well as HAR Title 16, Chapter 79.
The PRE-HEARING CONFERENCE will be conducted on September 27, 2017 at 9:00 a.m. in a hearings room at the Department of Commerce and Consumer Affairs, 335 Merchant Street, Suite 100, Honolulu, Hawaii 96813.
The HEARING will be conducted on December 7, 2017, at 9:00 a.m. in a hearings room at the Department of Commerce and Consumer Affairs, 335 Merchant Street, Suite 100, Honolulu, Hawaii 96813.
