YOU ARE HEREBY NOTIFIED that Plaintiff Department of Human Services, State of Hawai’i, has filed a Complaint and Summons against you for relief in Civil No. 16-1-0904-05 RAN in the Circuit Court of the First Circuit, State of Hawai’i, arising out of an action to recover overpayments, damages and civil penalties that were paid by the Hawaii medical assistance program for nonemergency transportation, allegedly sustained.
YOU ARE HEREBY SUMMONED to appear in the courtroom of the Honorable Rhonda A. Nishimura, at 777 Punchbowl Street, Honolulu, HI 96813, on March 28, 2017 at 9:00 o’clock a.m., or to file an answer or other pleading in response to the Complaint and Summons and serve it before said date upon Michael S. Vincent, Gary K. Senaga and Lyle T. Leonard, Deputy Attorneys General, attorneys for Department of Human Services, State of Hawai’i, whose address is 425 Queen Street Room 212, Honolulu, Hawai’i 96813. If you fail to do so, Judgment by Default will be taken against you for the relief demanded in the Complaint.
