REQUEST FOR PROPOSALS

The City and County of Honolulu, Department of Community Services (“DCS”), is seeking proposals from qualified, private nonprofit organizations to lease City properties to provide housing for persons with special needs. The properties are identified as follows:

1. LOWELL PLACE
Location: 1027 Lowell Place
Honolulu, Hawaii 96817
TMK: 1-6-006: 080

Improvements: A two story duplex, ten bedroom residential building with a garage. The building is of masonry and wood frame. Parcel is 5,271 square feet.

Use: Permanent housing for elderly.
2. HALOA DRIVE
Location: 1322 Haloa Drive
Honolulu, Hawaii 96818
TMK: 9-9-047: 081
Improvements: A wood frame, single family dwelling consisting of four bedrooms, two bathrooms, and a two car garage. Parcel is 6,180 square feet.
Use: Transitional housing group home for youth.
3. MAKUHINE STREET
Location: 1828 Makuahine Street
Honolulu, Hawaii 96817
TMK: 1-6-028: 040
Improvements: A one story, single family dwelling consisting of three bedrooms and three bathrooms, three office spaces, a dining room, two living rooms, a laundry, and two kitchen areas. Parcel is 8,414 square feet.
Use: A shelter for runaway and homeless youth.
4. TRANSITIONAL APARTMENTS
Location: Confidential site on Oahu
Improvements: A three story apartment building containing six (6) two bedroom, two bathroom units; one unit is reserved for the resident manager. Parcel is 6,800 square feet.
Use: Transitional housing for victims of domestic violence and their children.
5. KUNAWAI LANE
Location: 710 Kunawai Lane
Honolulu, Hawaii 96817

Improvements: A five (5) bedroom, two bath, single family detached dwelling. Parcel is 3,212 square feet.
Use: Permanent housing for elderly.
6. MAKAALOA STREET
Location: 91-1015 Makaaloa Street
Ewa Beach, Hawaii 96706

Improvements: A single family, two story dwelling containing five (5) bedrooms, three baths and a garage. Parcel is 3,724 square feet.
Use: Transitional housing for homeless youth.
7. ALEWA DRIVE
Location: 1816 Alewa Drive
Honolulu, Hawaii 96817
Improvements: A two story, 6 bedroom, 2.5 bath respite center with staff offices and two multi-purpose activity rooms. Parcel is 29,103 square feet.
Use: The property may be used as either: a) a senior community center, b) a senior day care center, c) a senior assisted living or residential facility (group home), d) a respite center for persons with Alzheimer’s Disease and their families, or e) other special needs uses to be considered by the City.

Preference will be given to those proposals that do not require displacement of clientele. Consideration will NOT be given to proposals requesting assistance for expenses required to carry out the regular responsibilities and operations of the proposing organization.

Selection of organizations to lease the properties and the lease are subject to the approval of the City Council. The properties were assisted with funds fromthe Community Development Block Grant (CDBG) program and/or the HOME Investment Partnerships (HOME) program and thus are subject to certain restrictions on use and management which are more fully described in the full Request for Proposals.

A Public Informational Meeting will be held on May 18, 2016 at the Department of Community Services, 715 South King Street, Second Floor Conference Room 205, Honolulu, Hawaii 96813 at 10:00 am.

Requests for clarifications or interpretations of any provision in the RFP may be submitted by a person in writing and delivered, mailed, emailed, or faxed to the Department of Community Services no later than 2:00 pm on June 03, 2016.

Interested parties may obtain a request for proposals packet at no charge from the Department of Community Services, 715 South King Street, Suite 205, Honolulu, Hawaii 96813. Proposals must be submitted no later than 2:00 pm on June 16, 2016.
