REQUEST FOR PROPOSALS

The City and County of Honolulu, Department of Community Services (“DCS”), is seeking proposals from qualified, private nonprofit organizations to lease City properties to provide housing for persons with special needs. The properties are identified as follows:
1. MALUNA STREET
Location: 1068 Maluna Street
Honolulu, Hawaii 96818

Use: Teaching family home for troubled youth.
2. OHIOHI PLACE
Location: 87-223 Ohiohi Place
Waianae, Hawaii 96792
TMK: 8-7-039: 009
Use: Transitional housing group home for persons with serious mental illness.
3. HALEIWA ROAD
Location: 66-528 Haleiwa Road
Haleiwa, Hawaii 96712
TMK: 6-6-013: 011
Use: A youth shelter providing residential services f or runaway, homeless, and other youth in crises.
4. RENTON ROAD
Location: 91-1259 Renton Road
Ewa, Hawaii 96707
TMK: 9-1-126: 006
Use: Operate a group home for youth aged 12 to 22.
5. MANU’U STREET
Location: 87-699 Manu’u Street
Waianae, Hawaii 96792
TMK: 8-7-024: 030
Use: Permanent supportive housing group home for persons with serious mental illness.

A Public Informational Meeting will be held on May 19, 2015 at the Department of Community Services, 715 South King Street, Second Floor Conference Room 205, Honolulu, Hawaii 96813 at 10:00 am. Interested parties may obtain a request for proposals packet at no charge from the Department of Community Services, 715 South King Street, Suite 205, Honolulu, Hawaii 96813.
