REQUEST FOR PROPOSAL
DENTAL SERVICES FOR THE HAWAII YOUTH CORRECTIONAL FACILITY
RFP No. HMS-503-08-04
Notice is hereby given that pursuant to Chapter 103F, Hawaii Revised Statutes, relating to purchases of health and human services, the Department of Human Services, Office of Youth Services (OYS) is soliciting proposals for Dental Services for Youth Incarcerated at the Hawaii Youth Correctional Facility.
The contract term begins on October 1, 2007 and terminates on September 30, 2009. Contract may be multi-term and may be extended, at the discretion of the OYS, up to 2 additional 12-month periods up to a maximum of 4 years.
Request for Proposals (RFP) for the above RFP number and title may be picked up or mailed upon request or downloaded on the web at http://www.hawaii.gov/spo2/ health/rfp103f/ beginning April 25, 2007.
The original and 3 copies of each proposal shall be mailed and postmarked by the United States Postal Service (USPS) on or before midnight, Hawaii Standard Time (HST) May 31, 2007, or hand delivered no later than 4:30 p.m., Hawaii Standard Time (HST), on May 31, 2007, at the drop-off site designated on the Proposal Mail-in and Delivery Information Sheet. Deliveries by private mail services such as Federal Express (FedEx) and the United Parcel Service (UPS) shall be considered hand deliveries. Proposals postmarked or hand delivered after the submittal deadline shall be considered late and rejected. There are no exceptions to this requirement. Incomplete and late proposals will be returned without action. Funding is subject to availability of funds and budget execution policies.
There will be an RFP orientation session on Tuesday, May 8, 2007 from 9:00 a.m. to 11:00 a.m. at the Office of Youth Services, 820 Mililani Street, Suite 817, Honolulu, Hawaii. All prospective applicants are encouraged to attend the orientation.
