RECONSIDERATION COMMITTEE

The Reconsideration Committee of the Agency will hold a public meeting on March 2, 2007, at 9:00 a.m., State Office Tower (Leiopapa A Kamehameha Building), at 235 South Beretania Street, Room 204, Honolulu, Hawaii. The purpose of this meeting will be to determine whether good cause has been shown, as provided in Hawaii Revised Statutes 323D-47 and Hawaii Administrative Rules 11-186-82, for the convening of a public hearing for reconsideration of the Agency’s decision on Certificate of Need Application #06-24A from Liberty Dialysis-Hawaii, LLC, for the relocation of four hemodialysis stations from Sullivan Dialysis (2230 Liliha Street, Honolulu) to 3638 Waialae Avenue, Honolulu, Hawaii, and for the addition of eight new hemodialysis stations at 3638 Waialae Avenue, Honolulu, Hawaii, at a capital cost of $3,120,000.
