YOU ARE HEREBY NOTIFIED THAT the State of Hawaii, Department of Commerce and Consumer Affairs, Plaintiff, has commenced an action against you in Civil No. 05-1-1569-09 (RKOL) for damages in the above-entitled Court wherein it requests judgment by way of injunction, civil penalties, and restitution in an amount to be determined, together with such other relief as the Court deems just.
YOU ARE HEREBY SUMMONED to appear before the Honorable Randal K.O. Lee, Judge of the above-entitled Court, in his Courtroom at Kauikeaouli Hale, 1111 Alakea Street, Honolulu, Hawaii 96813 on Tuesday, the 23rd day of January, 2007, at 9:00 oclock a.m., of said day, or to file an answer or other pleading and serve it before said day upon Joseph W. Lee, Plaintiffs attorney whose address is 235 S. Beretania Street, 9th Floor, Honolulu, Hawaii 96813. If you fail to do so, judgment by default will be taken against you for the relief demanded in the complaint.
