REQUEST FOR CONTRACTOR QUALIFICATIONS
The YMCA of Honolulu (YMCA) hereby invites interested and qualified contractors to submit a statement of qualifications for construction services for rock fall mitigation improvements for the Waianae Coast YMCA. Based on the evaluation criteria specified below, YMCA will invite contractors to submit detailed proposals.
PROJECT DETAILS:
Location: 86-071 Leihoku Street, Waianae, Hawaii, 96792, TMK 1-6-01:61.
General Scope of Work includes but not limited to:
1. Site Work including grading.
2. Clearing of debris from existing drainage canal.
3. Construction of rock fall mitigation fencing.
4. Overall budget for the project is $600,000 – $750,000.
5. Drawings by Hida/Okamoto and Associates and Geolabs are available for duplication at HonBlue at contractors expense or for viewing at YMCA offices located 1441 Pali Highway, The Harry & Jeanette Weinberg Building, 3rd Floor.
6. Final bid set drawings will be available for duplication at HonBlue from January 2007 at contractors expense.
Anticipated Start of Construction: March 2007 for completion by June 15, 2007.
MINIMUM QUALIFICATIONS:
1. Must have at least three (3) years of operating experience under the current company organization.
2. Must have an established office in Honolulu.
3. Must have required licenses to perform the work in accordance with Chapter 444 HRS.
4. Must be in good standing in the State of Hawaii.
PROPOSAL EVALUATION:
The following criteria will be used to evaluate all submissions:

1 Evidence of companys ability to perform the work, as 20%
evidenced by profiles of key staffs professional and
technical competence, experience, and facilities.
2 Workload, financial strength and capacity to perform the 20%
work.
3 Past performance in terms of cost control, quality of work, 40%
compliance to schedules and knowledge of local building
practices.
4 Evidence of previous experience constructing similar type 20%
of projects.
5. Evidence of compliance with minimum qualifications. Mandatory
STATEMENT OF QUALIFICATIONS SHALL INCLUDE:
1. A letter of interest.
2. The name of the company, the principal place of business and location of all its offices.
3. The age of the company and its average number of employees over the past year.
4. The names, years of company service, training and qualifications of key members of the company.
5. The names and phone numbers of up to five clients who may be contacted and at least two for whom services were rendered during the preceding year.
6. Documentation to substantiate each of the listed evaluation criteria.
7. Any promotional or descriptive literature that the company desires to submit.
8. Completed AIA Document A305 Contractors Qualification Statement.

Date: Wednesday, December 27, 2006
Qualifications received after this time and date will be rejected. Honolulu mail-in deliveries must be postmarked prior to noon, 12/25/06. Out-of-state mail-ins must be postmarked prior to noon, 12/22/06.
All qualifications must be submitted in duplicate (2) and must clearly indicate the name of the project.
