REQUEST FOR QUALIFICATIONS

Qualified professional design firms are invited to submit a letter of interest and Statement of Qualifications (SOQ) for the planning and design of a warehouse/training center at Lanakila Rehabilitation Center, Inc., Halawa Warehouse Facility, 99-1132 Iwaena St, Aiea, Oahu. TMK: 9-9-073:006 gross building area 21900 sq. ft. The project is intended to provide warehouse and training facilities for adults with disabilities and shall include: elevator, warehouse space, multi-purpose rooms, classroom training areas, office, storage and restroom facilities. Tentative start date is March 30, 2006. Due to federal funding made available through City and County of Honolulu, the project will be subject to local, State and federal statutes.

Pertinent information and evaluation criteria:

1. Name and age of firm, principal place of business and average number of employees over the past 5 years.
2. Professional qualifications of firm and sub consultants including education, training and experience of key personnel.
3. Ability to provide professional services in a timely manner: including quality control procedures, scheduling capabilities, capacity and capability to perform work.
4. Past performance in terms of cost control, quality of work and compliance with performance schedules.
5. Familiarity with federal funding. Please provide examples.
6. Familiarity with Halawa area and project location.
7. Demonstrated knowledge of local codes and ADA Accessibility Guidelines.
8. Names and phone numbers of five clients who may be contacted including at least two (2) for whom services were rendered during the preceding year.
9. Professional Errors and Omissions insurance of at least $1,000,000.
10. Not currently or at any time in the past on the Federal or State list of debarred, disqualified or ineligible contractors or consultants.

The letter of interest and three (3) copies of the Statement of Qualifications, limited to no more than ten (10) single-sided pages, must be received by LRC no later than 4:00 p.m. on March 13, 2006.
