PUBLIC NOTICE

The Department of Health, State of Hawaii, is notifying all interested persons of the proposed settlement between the Department of Health and G. Ibara Heavy Equipment Rentals. The Department of Health has issued a Notice and Finding of Violation and Order (NOVO), in Docket No. 13-CA-EO-16, alleging that G. Ibara Heavy Equipment Rentals violated state air pollution laws, HRS §342B-11, HAR §11-60.1-2, and Noncovered Source Permit No. 0447-01-NT by failing to meet various Monitoring, Reporting, and Annual Fee requirements.
A Consent Order (CO) has been drafted that proposes to settle this case. The proposed CO and NOVO is available for public review during regular office hours, Monday through Friday, 7:45 a.m. to 4:15 p.m., at the following locations:
Oahu: Clean Air Branch, Department of Health
919 Ala Moana Boulevard, Room 203, Honolulu
Maui: Maui District Health Office, Department of Health
54 High Street, Room 300, Wailuku
