PUBLIC NOTICE
INTENTION TO DIRECT LEASE

Pursuant to Sections 171-16(c) and 171-43.1, Hawaii Revised Statutes, the
Department of Land and Natural Resources hereby gives notice of its intent to directly lease 5.00 acres of State land to thePuna Community Medical Center situated at Keonepoko Nui, Puna, Hawaii; and, identified as TMK: (3) 1-5-008: 005. This lease will be for a period of 65-years at an initial annual rent of $480.00, and shall enable the Lessee to utilize said parcel for a comprehensive medical center.

Information related to this proposed direct lease may be reviewed during office
hours at the Department of Land and Natural Resources, Land Division located at 75 Aupuni St., Rm. #204, Hilo, Hawaii; and, at the offices of the Land Division on the Islands of Kauai, Maui, and Oahu. Please call, or write to any of these offices to request information in an alternate format.
