NOTICE TO FOOD SERVICE MANAGEMENT COMPANIES
REGISTER WITH HAWAII CHILD NUTRITION PROGRAMS
FOR CONTRACTING WITH A CHILD NUTRITION PROGRAM IN THE STATE OF HAWAII
Notice is hereby given to Food Service Management Companies to complete an application for registration and submit it for approval by Hawaii Child Nutrition Programs for the purpose of contracting with institutions operating United States Department of Agriculture Child Nutrition Programs in the State of Hawaii.
The application for registration is available online at http://hcnp.hawaii.gov/wp-content/uploads/2014/04/FSMC-Application.doc or by calling Hawaii Child Nutrition Programs at 808-587-3600.
Submit the fully completed application for registration by 5/30/14. The form can be submitted by email to sarah_kottek@notes.k12.hi.us, or via US mail to:
