The Agency has received a request for a public information meeting on certificate of need application #13-12A for administrative review from Kokua Ambulance, LLC for the establishment of back-up surface ambulance services for the Honolulu Emergency Medical Services Division to be located at 501 Sumner Street, 6E, Honolulu, HI, at a capital cost of $310,000. The meeting will be held on December 10, 2013, 10:00 a.m., State Office Tower (Leiopapa) Building, 235 South Beretania Street, Room 204, Honolulu, Hawaii. Certificate applications are reviewed according to the following criteria: relation to the state health services and facilities plan, need and accessibility, quality, cost and finances, relation to the existing health care system, and availability of resources.
