The Agency has received certificate of need application #13-11 for standard review from Hawaiian Healthcare Corporation dba ProHealth Hospice & Home Care, for the establishment of home health agency and hospice services at 500 Ala Moana Blvd., Suite 5-325, Honolulu, HI, at a capital cost of $202,030. Application reviews are scheduled as follows:

November 21, 2013, Statewide Health Coordinating Council, 9:00 a.m., State Office Tower (Leiopapa) Building, 235 South Beretania Street, Room 204, Honolulu, HI.
