You are hereby summoned to appear in the courtroom of the Presiding Judge of the South Kohala District Family Court, Waimea Courthouse, Waimea Civic Center, 67-5187 Kamamalu Street, Kamuela, Hawaii, 96743, on the 8th day of July, 2013 at 10:30 o’clock a.m. for a return date and adjudication hearing.

YOU ARE HEREBY NOTIFIED THAT YOUR PARENTAL AND CUSTODIAL DUTIES AND RIGHTS CONCERNING THE CHILD WHO IS THE SUBJECT OF THE PETITION FOR FOSTER CUSTODY MAY BE TERMINATED IF YOU FAIL TO APPEAR ON THE DATE SET FORTH IN THIS SUMMONS OR TO SUBMIT A WRITTEN RESPONSE TO THE COURT.
