The Agency has received certificate of need application #13-04 for standard review from Kokua Ambulance, LLC for the establishment of Surface Ambulance services at 1020 Auahi Street, Bldg. 2, Bays 1 and 2, Honolulu, HI, at a capital cost of $1,150,000. Application reviews are scheduled as follows:
May 13, 2013, Oahuwide Certificate of Need Review Committee, 11:00 a.m., State Office Tower (Leiopapa) Building, 235 South Beretania Street, Room 204, Honolulu, Hawaii;
May 23, 2013, Statewide Health Coordinating Council, 9 a.m., State Office Tower (Leiopapa) Building, 235 South Beretania Street, Room 204, Honolulu, Hawaii.
