NOTICE
INVITATION FOR BIDS

Pursuant to Chapter 103D, HRS, SEALED TENDERS will be received up to and opened at 2:00 P.M., Hawaiian Standard Time (HST) on May 27, 2009 in the Office of the Division of Purchasing, Department of Finance, 4444 Rice Street, Room 303, Lihue, Kauai, Hawaii for the furnishing and delivery of AUTOMATED EXTERNAL DEFIBRILLATORS, PHILLIPS HEARTSTART M3861A AND M5066A for the County of Kauai Fire Department and the Department of Parks and Recreation, Lihue, Kauai, Hawaii. Bids received after the date and time specified above shall be rejected.
The Director of Finance also reserves the right to reject any or all bids, in whole or in part, if deemed to be in the best interest of the County.
Bids must be signed in ink by the person or persons duly authorized to sign bids in the space provided for signature on the bid form.
BIDDERS ARE HEREBY NOTIFIED THAT EVIDENCE OF THE AUTHORITY OF THE PERSON(S) SIGNING THE BID DOCUMENT IS REQUIRED TO BE INCLUDED WITH THE BID DOCUMENTS. FAILURE TO COMPLY WITH THIS REQUIREMENT WILL BE CAUSE FOR REJECTION OF THE BID AS BEING NON-RESPONSIVE.
The successful bidder is required to submit to the County of Kauai, a tax clearance from the State Department of Taxation and the U.S. Internal Revenue Service, a Certificate of Good Standing from the State Department of Commerce and Consumer Affairs, and a Certificate of Compliance from the State Department of Labor and Industrial Relations as a prerequisite to entering into a contract.
Solicitation documents may be obtained from the Division of Purchasing, phone (808)241-4288 upon application of Invitation For Bid Document No. 3022. Bids must be submitted on County issued forms.
