Pursuant to Section 323D-44.5, Hawaii Revised Statutes, the Agency has received a request for a public information meeting on the following certificate of need application: application #08-23A for administrative review from Rainbow Dialysis, LLC, for the establishment of outpatient Hemodialysis and Peritoneal Dialysis at Kaiser Permanente Wailuku Clinic, 80 Mahalani Street, Wailuku, Hawaii, at a capital cost of $1,109,852. The meeting will be held on Tuesday, February 17, 2009, 11:30 am, at the J. Walter Cameron Center Auditorium, 95 Mahalani Street, Wailuku, Maui.
Certificate applications are reviewed according to the following criteria: relation to the state health services and facilities plan, need and accessibility, quality, cost and finances, relation to the existing health care system, and availability of resources.
