YOU ARE HEREBY NOTIFIED that you are to contact the Department of Hawaiian Home Lands\u00D5, regarding the disposition of your Residential Lease No. 6816, Lot No. 3, situate at Waiakea, Hilo, Hawaii, and further identified as TMK: 3-2-2-063:001.

Should you fail to respond to this notice; the Department of Hawaiian Home Lands will request that the Hawaiian Homes Commission approve the scheduling of a Contested Case Hearing to afford you the opportunity to present your case as to why your
Residential Lease No. 6816 should not be cancelled.
