YOU ARE HEREBY NOTIFIED THAT State of Hawaii, Department of Commerce and Consumer Affairs, Plaintiff, has commenced an action in Civil No. 08-1-1438-07 EEH for damages suffered against you in the above-entitled Court wherein it requests judgment by way of an injunction, civil fines and restitution in an amount to be determined at trial, together with such other relief as the Court deems just.
YOU ARE HEREBY SUMMONED to appear before the Honorable Eden E. Hifo, Judge of the above-entitled court, at 777 Punchbowl Street, Kaahumanu Hale, Honolulu, Hawaii, on Tuesday, the 3rd day of February, 2009, at 9:00 o’clock a.m., of said day, or to file an answer or other pleading and serve it before said day upon Wendy J. Utsumi, Plaintiff’s attorney, whose address is 235 South Beretania Street, Ninth Floor, Honolulu, Hawaii 96813. If you fail to do so, judgment by default will be taken against you for the relief demanded in the complaint.
